clinician drew up 1.3 ml of diluent, added it to the vaccine vial, then drew up 0.2 ml, which she administered to the patient. however, after giving the vaccine, she drew up the remaining doses and found that only a total of 6 doses could be drawn up from the vial (not the anticipated 10 doses she should have gotten). parents were notified on 2/4/2022. parents reported that patient had minimal side effects of the vaccine and parents declined the offer to repeat the dose in case she was under-dosed or given saline only
