third dose: pfizer.; third dose: pfizer; left side, occurred, and her cheek felt paralyzed; four or five weeks later, herpes zoster has exploded; her left corner of the mouth hang down; pain in her arm; unpleasant feeling of currents in her head; tiredness; swelling of her eye lid; this is a spontaneous report received from a contactable reporter(s) (consumer or other non hcp) from the regulatory authority-web. other case identifier(s): at-astrazeneca-2022a048624 (astrazeneca). a 77 year-old female patient received bnt162b2 (comirnaty) (batch/lot number: unknown) as dose 3 (booster), single for covid-19 immunisation. relevant medical history included: "herpes zoster" (unspecified if ongoing), notes: problems with herpes zoster. the patient's concomitant medications were not reported. vaccination history included: vaxzevria (dose 1, single), for covid-19 immunisation; vaxzevria (dose 2, single), for covid-19 immunisation. the following information was reported: off label use (medically significant), outcome "unknown", described as "third dose: pfizer."; interchange of vaccine products (medically significant), outcome "unknown", described as "third dose: pfizer"; facial paralysis (medically significant), outcome "unknown", described as "left side, occurred, and her cheek felt paralyzed"; herpes zoster (medically significant), outcome "unknown", described as "four or five weeks later, herpes zoster has exploded"; facial asymmetry (medically significant), outcome "unknown", described as "her left corner of the mouth hang down"; pain in extremity (non-serious), outcome "unknown", described as "pain in her arm"; head discomfort (non-serious), outcome "unknown", described as "unpleasant feeling of currents in her head"; fatigue (non-serious), outcome "unknown", described as "tiredness"; swelling of eyelid (non-serious), outcome "unknown", described as "swelling of her eye lid". clinical course: on an unknown date, the patient experienced left side, occurred, and her cheek felt paralyzed,her left corner of the mouth hang down, four or five weeks later, herpes zoster has exploded, pain in her arm, swelling of her eye lid, tiredness and unpleasant feeling of currents in her head. sender comment: facial paralysis, facial asymmetry and herpes zoster are not listed in the company core data sheet of azd1222. the events could be associated with each other. medical history of herpes zoster could be considered as a risk factor for the events. due to limited information on circumstances leading to the events, baseline health status before vaccination, exact date of vaccinations and onset of events, other relevant medical and family history (head trauma, tumour, stroke, diabetes, autoimmune disease), concomitant medications, clinical course, treatment given, detailed etiological and diagnostic work-up (physical and neurological examination, electromyography, imaging studies, tzanck smear, serologic and polymerase chain reaction test), the evaluation did not find evidence to suggest a causal relationship between the events and azd1222. no follow-up attempts are possible; information about lot/batch number cannot be obtained. no further information is expected
