lack of strength; headache; pain; joint pain; impaired her mobility; feeling of being bruised; waking up in the night in pain; lack of vit d; muscles felt strained; stiffness, stiff in both her arms (mainly shoulders and legs (mainly hip areas); muscle pain; off label use; interchange of vaccine products; this is a spontaneous report received from a contactable reporter(s) (consumer or other non hcp) from the regulatory agency (ra). the reporter is the patient. regulatory number: gb-mhra-webcovid-202202171256453120-kpp7o (ra). other case identifier(s): gb-mhra-adr 26612765 (ra). a 63 year-old female patient received bnt162b2 (comirnaty), administration date 10dec2021 (lot number: fm3802) as dose 3 (booster), single for covid-19 immunisation. relevant medical history included: "asthma" (unspecified if ongoing). the patient's concomitant medications were not reported. vaccination history included: covid-19 vaccine astrazeneca (dose 1, batch/lot no: 4120z002), administration date: 13mar2021, for covid-19 immunization, reaction(s): "headache"; covid-19 vaccine astrazeneca (dose 2), administration date: 02may2021, for covid-19 immunization; flu vaccine , for immunization, reaction(s): "no aftereffects ". the following information was reported: off label use (medically significant) with onset 10dec2021, outcome "unknown", described as "off label use"; vitamin d deficiency (medically significant), outcome "unknown", described as "lack of vit d". the events "off label use", "interchange of vaccine products", "lack of strength", "headache", "pain", "stiffness, stiff in both her arms (mainly shoulders and legs (mainly hip areas)", "muscle pain", "joint pain", "muscles felt strained", "impaired her mobility", "feeling of being bruised", "waking up in the night in pain" and "lack of vit d" were evaluated at the physician office visit. the patient underwent the following laboratory tests and procedures: blood test: negative, notes: negative but did show lack of vit d; sars-cov-2 test: no - negative covid-19 test. therapeutic measures were taken as a result of asthenia, headache, pain, musculoskeletal stiffness, myalgia, arthralgia, muscle strain, mobility decreased, contusion. additional information: suffered with asthma treated with preventer inhaler. this was well managed and did not cause any problems in daily activities. patient has not had symptoms associated with covid-19. patient has not tested positive for covid-19 since having the vaccine. patient was not enrolled in clinical trial. the day after she had booster vaccine, her muscles felt strained and stiff in both her arms (mainly shoulders and legs, mainly hip areas). she took some ibuprofen. gradually over the next few weeks this became worse with a feeling of being bruised and stiffness which impaired her mobility (getting dressed became difficult) she began taking more ibuprofen and paracetamol just to keep moving. she was waking up in the night in pain if she turned over in bed. there was a lack of strength in her arms (even pressing the alarm button took two hands one supporting the other). this was a first time experience. her mobility has always been good and she had not suffered with joint pain before. she let her gp know in february 2022. he did blood tests for polymialgia. this was negative but did show lack of vit d. it just seemed a coincidence that this started the day after the third dose of vaccine. she had the flu vaccine in september with no aftereffects and only a short headache after the first astrazeneca vaccine. the joint pains were thankfully easing off now but she still need some pain relief most days. she thought it worth reporting as talking to friends a few have experienced similar things. relevant investigations or tests conducted: blood test was taken by gp. case reported as serious by health authority (other medically important condition). no follow-up attempts are possible. no further information is expected
