heart attack; heart pain; this is a spontaneous report received from a contactable reporter(s) (consumer or other non hcp) from the regulatory agency and product quality group. a 55 year-old female patient received bnt162b2 (comirnaty) (batch/lot number: unknown) as dose 4 (booster), single for covid-19 immunisation. relevant medical history included: "bronchiectasis" (unknown if ongoing); "diabetes mellitus" (unknown if ongoing); "steroid therapy" (unknown if ongoing), notes: taking regular steroid treatment (e.g. orally or rectally); "sarcoidosis" (unknown if ongoing); "cardiomegaly" (unknown if ongoing); "high cholesterol" (unknown if ongoing); "hypertension" (unknown if ongoing). the patient took concomitant medications. vaccination history included: bnt162b2 (dose 1 , batch/lot number-ej0724), administration date: 17dec2020, for covid-19 immunisation, reaction(s): "pericarditis", "drug ineffective", "sars-cov-2 infection", "myocarditis", "heart racing", "heart rate high", "fatigue/unusual tiredness", "chest pain", "fever", "shortness of breath", "racing heart (tachycardia)", "cardiomegaly"; bnt162b2 (dose 2), for covid-19 immunisation; bnt162b2 (dose 3), for covid-19 immunisation. the following information was reported: myocardial infarction (medically significant), outcome "unknown", described as "heart attack"; angina pectoris (medically significant), outcome "unknown", described as "heart pain". the event "heart pain" was evaluated at the physician office visit. the patient underwent the following laboratory tests and procedures: blood cholesterol: (unspecified date) high; blood pressure measurement: (unspecified date) high; blood test: (unspecified date) unknown results; computerised tomogram: (unspecified date) unknown results; heart rate: (27dec2020) high; sars-cov-2 test: (03jan2021) positive, notes: yes - positive covid-19 test; x-ray: (unspecified date) enlarged heart, notes: diagnosed with cardiomegaly. therapeutic measures were taken as a result of myocardial infarction, angina pectoris. clinical course: 4th pfizer - 2 days after receiving heart pain felt like heart attack - paramedics called treated as heart attack, bt/xray done - seems like a reaction to the vaccine. the patient had condition sarcodosis (lungs). unsure if patient was enrolled in clinical trial. the report was related to possible inflammation of the heart (myocarditis or pericarditis). the symptoms did not lead to a hospital stay. the diagnosis was made by a medical professional (respiratory / cardiologist). the treatment with ivabradine was given. it was unknown if blood tests, such as for certain proteins (called troponin) that signal heart muscle damage was taken. no follow-up attempts are possible; information about lot/batch number cannot be obtained. no further information is expected.; sender's comments: linked report(s) : gb-pfizer inc-202200256611 due to same patient/drug/reporter, different dose number and event
